Aspectos Clínicos, Diagnósticos e Terapêuticos da Síndrome de Arnold-Chiari Tipo II

Introdução: A síndrome de Arnold Chiari, também conhecida como malformação de Arnold Chiari, foi definida pela primeira vez em 1890 pelo patologista austríaco Hans Chiari. É um grupo de malformações congênitas que afetam o Sistema Nervoso Central (SNC), especialmente a fossa craniana posterior e do rombencéfalo, e geralmente está associada à mielomeningocele. Objetivos: Este estudo visa revisar os aspectos clínicos, diagnósticos e terapêuticos da síndrome de Arnold-Chiari Tipo II. Metodologia: Trata-se de uma revisão bibliográfica com abordagem qualitativa exploratória e descritiva, foi realizada em agosto de 2024, a pesquisa utilizou as bases SciELO e PubMed com os termos de busca "Arnold Chiari Syndrome Type II ". Foram incluídos artigos originais, gratuitos, em inglês e português, sem limite de tempo, desde que relevantes para o tema. Resultados: A malformação de Arnold Chiari tipo II é caracterizada pela herniação do tronco cerebral e cerebelo através do forame magno. Geralmente está associada à disrafismo espinhal/mielomeningocele. A compressão das estruturas cerebrais causa os sintomas observados nos pacientes, embora alguns possam ser assintomáticos, a cefaleia occipital durante manobras de Valsalva é o sintoma mais comum. Outros sintomas incluem vertigem, parestesia, dificuldades de equilíbrio. Conclusões:  O tratamento varia de acordo com a gravidade e as características clínicas, muitas vezes requerendo intervenção cirúrgica para remodelar e descomprimir a fossa craniana posterior malformada, embora o risco de sintomas residuais após a cirurgia seja alto. Esses sintomas residuais podem ser paliados farmacologicamente por meio de AINEs ou relaxantes musculares, dependendo da necessidade e da resposta individual do paciente.
