VASOPRESSORES MAIS UTILIZADOS NAS CESÁREAS E SEUS EFEITOS MATERNOS E FETAIS: REVISÃO INTEGRATIVA

A hipotensão arterial materna durante a cesariana sob anestesia raquidiana representa uma das complicações mais frequentes no contexto obstétrico, estando associada a repercussões maternas e fetais potencialmente significativas. Nesse cenário, os vasopressores desempenham papel fundamental na manutenção da estabilidade hemodinâmica da gestante, sendo amplamente utilizados na prática anestésica. O presente trabalho tem como objetivo analisar os vasopressores mais utilizados durante cesarianas, destacando seus mecanismos de ação, eficácia clínica, efeitos adversos maternos e repercussões neonatais. Trata-se de uma revisão integrativa, desenvolvida a partir da análise de artigos científicos publicados em bases de dados como Pubmed, Scielo, Google Scholar e Biblioteca Virtual em Saúde (BVS), no período entre 2015 e 2026. Foram utilizados descritores como “vasopressores”, “cesárea”, “hipotensão materna”, “anestesia raquidiana”. Foram analisados 31 estudos, cujos resultados evidenciam que a fenilefrina permanece como o vasopressor de primeira escolha em muitas diretrizes devido à sua eficácia no controle da hipotensão e à menor incidência de acidose fetal. Entretanto, medicamentos como efedrina, norepinefrina e metaraminol também demonstram benefícios específicos, especialmente em determinadas condições clínicas maternas. Observou-se que cada vasopressor apresenta perfil farmacológico distinto, influenciando diretamente parâmetros hemodinâmicos, frequência cardíaca materna, débito cardíaco e condições neonatais. Conclui-se que a escolha do vasopressor ideal deve considerar fatores individuais da gestante, condições obstétricas e experiência da equipe anestésica, buscando garantir segurança materno-fetal e melhores desfechos clínicos.
