A abordagem terapêutica da psiquiatria na síndrome do intestino irritável

Introdução: A Síndrome do Intestino Irritável (SII) é um distúrbio gastrointestinal muito comum, caracterizado por dor ou desconforto abdominal de forma recorrente juntamente com mudança de hábitos intestinais. Embora as causas exatas da SII sejam desconhecidas, sabe-se que fatores psicológicos, como estresse e ansiedade, podem influenciar significativamente o desenvolvimento e a progressão da doença. A abordagem terapêutica da psiquiatria pode ser uma alternativa eficaz para o tratamento da SII. Objetivos: Apresentar os métodos terapêuticos e benefícios da abordagem psiquiátrica no tratamento da SII. Metodologia: Será realizada uma revisão bibliográfica da literatura para ampliar as informações acerca da abordagem psiquiátrica em pacientes com a SII. Para elaborar o corpus da pesquisa, serão realizadas consultas nas bases de dados científicas na internet: Biblioteca Virtual de Saúde (BVS) e Pubmed. Serão utilizados ainda os Descritores em Ciências de Saúde: Irritable Bowel Syndrome AND Psychiatry AND Treatment. Resultados: As estratégias atuais de tratamento para a SII concentram-se principalmente no alívio dos sintomas. As duas classes de antidepressivos que poderiam fornecer eficácia no tratamento dos sintomas da SII foram antidepressivos tricíclicos e seletivos inibidores da recaptação da serotonina. Alguns dados também mostram que os inibidores seletivos da recaptação de norepinefrina. Enquanto o modelo TCC para o tratamento da SII concentra-se principalmente nos seguintes componentes: I) psicoeducação sobre a resposta ao estresse e sua relação com os sintomas gastrointestinais; II) Construir insights sobre respostas cognitivas e comportamentais aos sintomas da SII e/ou medo dos sintomas; e III) Modificar essas respostas para diminuir o sofrimento relacionado à SII e diminuir a reatividade física ao estresse. O manejo de pacientes com SII é desafiador, uma vez que o diagnóstico e o tratamento podem exigir diversas abordagens com resultados insatisfatórios.
